Retrieve uploaded files names with Bluimp jquery

I want to use Blueimp to upload files, but I do not know how to modify the Html file in order to retrieve uploaded files names (in order to insert them into a MySQL database).

When I submit the form, I do not know how to modify the field 'action' in line 15 here:
